As a LEED Accredited Professional, Jane Rohde focuses on healthcare and senior living education, which includes ‘green’ building information and sustainable business case models.

A Day in the Life of Josephine

Aging in Place through Healthcare Case Management Innovation

By utilizing “A Day in the Life of Josephine” as a profile of an aging individual, this session demonstrates the need and preference for seniors to have the choice of Aging in Place by utilizing a Healthcare Case Management model. Project examples will be utilized demonstrating reduced mortality rates and provisions for continuum of care. Project examples include a Hope VI site, the East Capital Project, in Washington, DC, and the development of a retirement community model based in healthcare case management in the City of Boulder City, Nevada. The presentation will include a case study that outlines the culmination of research and information that has led to the development of a housing model that addresses the healthcare and financial needs of the residents of Boulder City.

This session provides an innovative view on the integration of affordable and economically viable care, while supporting the sustainability of independent lives for seniors. The model utilized for Boulder City has the opportunity for replication, assisting seniors and their families with Aging in Place through Universal Design principles and the Healthcare Case Management wellness model. The goal is to provide a model of care that is fully supported by the built environment for our parents’ futures, as well as our own.
